Roll For Adventure: Triumph or Tragedy - A roll and write game

This was for a personal game design challenge. Make a simple game using 2 D6 dice. It had to fit on one sheet of paper. 
I came up with the idea of this Roll and Write game. 

While D6 was the chosen dice, any pair of dice will work as long as both dice are the same type. 2D20, 2D10 etc. 

It's designed to be fairly simple to play. 

One file is one page that is just the base game. 

One file contains a story driven element with the same rules.
